I  built a macro flash bracket along the lines of John Shaw's "butterfly 
bracket"

I adapted a rear lens cap into a reversed lens ringflash threaded mount.

I bought a Wemberly macro flash bracket to replace the home-made one, 
but I still use the rear lenscap thingy.

For the bracket, cost.  For the reversed lens ring flash mount, I 
couldn't find one on the market.  Apparently they exist now.
