I can't recall the name or number of it. It's an hourglass shaped rubber mount with a threaded coupling on one side to fit on a monopod and a camera tripod mount screw on the other. It allows you to bend the setup flexibly, while allowing the monopod to take the weight of the camera and lens.

With a monopod, the best flexible attachment I've used is a little
rubber gizmo that Manfrotto sells. It doesn't allow portrait
orientation, but lets me move the camera quickly and easily for
panning at sports events, etc.
