I think that you only ever need to use a long lens on a monopod with no facility to alter the tilt once before you understand that it's pretty impractical. I use a Manfrotto ball, it's fine with camera, lens +TCs, just lock the ball tight and lock the QR so it can't be accidentally un-latched off.
