The closest thing that happened to me is what John (Francis) described: the lens was not fully locked and I began turning the zoom ring then realizing the whole lens was turning. That did not require pressing the lens release. That mistake can happen to any camera mounts (unlocked lenses).
Anyway, this whole conversation began because of an acquaintance arguing why his chose his brand of camera and not a Pentax; he had this fear of making that mistake. I have never made that mistake and I suspect that no one else has. Everyone's reply seems to have validated that. At a crossroad is my photographic journey, I had to decide whether to go Pentax or Nikon and my options were reduced to the FM and the MX whose features were more or less a match. It was the K-mount and its ergonomic arragement that drew me in.
