If there is a goodly amount of dust inside the lens, it will not be as sharp or as contrasty as it would if it were clean. The difficulty is finding someone competent to do a CLA and reassemble the lens properly. It's not simple with modern lenses, with older lenses it's easier. Of course, it's also pretty expensive to have it done right ... locally, they charge about $80 per hour for labor and have a 1 hour minimum.
