I can't really imagine why you'd want to use RAW+JPEG capture mode  
when you're using Lightroom. The JPEGs become redundant.

But that aside, I import from card using Lightroom's tools, convert  
to DNG on the fly, and put the converted files into a directory  
structure that looks like:

YYYY
   in-progress
       YYYYMMDD-label1
       YYYYMMDD-label2
       ...

I have been leaving the file names alone, as they come out of the  
camera, but lately I've begun to date-code them too. Instead of  
IMGPXXXX.dng they become YYMMDDGD-XXXX.dng. It seems to  make it  
easier to find things.

During import, I have a second copy of the RAW files, exactly as they  
are on the card, written to a separate, dedicated backup storage  
volume. Lightroom creates dated folders for this automatically.

Adding a set of keywords, and IPTC metadata as well, during import  
helps get a leg up on organizing too.
