Obtaining what is called "International Driving Permit" is rather simple.
You just go to any AAA center, and they will to it for you while you
wait (10 minutes once they start working with you).  
Last time I did it it costed $12 or $14 (I see it went up to $15 now.)
if you bring two photos with you, or they would take you photos for
extra $10. If you are a AAA member, one of these charges is discounted,
but you don't have to be a AAA member to get this document.

the National Auto Club) can also do it:

I heard about people who were able to get away with their US DL, but
I also heard about people who had dificulties.
Personally, when I went to Italy, - I got one. BTW, I rented a car 3 
times from Hertz which at that time was much less expensive than Avis 
or anything else we could find,- but only if you make reservations 
ahead of time, from the US; otherwise it is expensive.
