i think it varies quite a bit with artists, but many do improvisational sketches to work up ideas, then they choose the best ideas for further work; i think this has direct parallels in photography practice

and i personally know a couple of painters who spend hours, even weeks, covering up the first things they put on canvas with very different other things in an evolutionary process that is not at all planned from the start (this approach is not so easy to parallel in photography)

and consider music, which i think shows the breadth of how planned or unplanned good art can be
