I actually missed out some info, so I'll repost the previous info,
amended, and more basic.

Start on your Mac desktop. Create a new folder (in the Finder, File>New
Folder) OR if you already have a folder designated by iTunes as your
folder that iTunes syncs photos from, use that if you like. As long as
you know what folder is the folder iTunes uses to sync photos to/from
with your iPhone, and where it is on your Mac, then you can do the following:


On my Mac, (I run 10.6.8) I created a folder called 'iPhone Pics' and I
keep it [anywhere - on the desktop if you like].

I keep it out of the way in my home folder. But to make it easily
accessible, I dragged the icon of my iPhone Pics folder into a sidebar
of an open window. (To organise the sidebar better, in the Finder go to
Finder>Preferences and see the 'sidebar' button to tidy up). This way,
every time I open a new window, my iPhone Pics folder is right there on
the left in the sidebar. I click once on that and then I can see the
contents of the iPhone Pics folder, and add to it or delete things.

To tell iTunes that I wanted the 'iPhone Pics' folder to be the default
folder so that every time I sync my iPhone with my Mac, it uses 'iPhone
Pics as the one single folder - the master pics folder, if you like -
you simply have iTunes open with the iPhone plugged up by USB, and in
the bar on the left of the iTunes window, select 'iPhone', and then see
the tabs on the right, click on 'photos'. The first option is for
selecting a folder to sync photos with. I navigated to my iPhone Pics
folder, and now that is set as the default folder for syncing all photos
on my iPhone.

Incidentally you can create folders within that folder, and so I have
many folders separating subjects like one for landscapes, one for
portraits etc.

Then it's just a case of dropping jpegs into those folders straight on
my mac desktop and syncing the iPhone.

A few notes: I only ever sync locally to my mac, I don't use iCloud.
Also, the folder and pics in those folders managed via the mac desktop
can't be edited while on the iPhone. EG, if you want to delete a pic,
you do it on the Mac, and then sync the phone.
