Were the errant footprints placed via the schematic object call, or were they placed manually? That is, are the schematic and PCBs synced, or simply related? If it's yours, I'm guessing that it should have been, at least by your surprise, but those who manually route do not always concern themselves with following "PCB designer" canon, and so such a PCB may be constructed from primitives rather than by formal method. If you're dealing with someone else's work, then this _might_ be the case.
I have designed many PCBS over the years in which the electrical schematic actually followed design of the PCB, and I'm pretty sure there are others who have also done so. Sometimes it's simply more expedititious to get the PCB laid out and etched, rather than to draw inside the lines.
