Erik,
        like any non-standard feature you simply must identify the feature and 
specify what you want done with it. I normally do this on a drill/fab drawing 
but it can be done anywhere, on any layer, in any manner that you find most 
useful or convenient. I would also use some note in a readme file just to call 
attention to this special detail as well.
        Maybe discuss this with you fabricator and have them give you ideas of 
how their other customers have done similar features in the past. In short 
there is no singular standard, just go with what will reliably work for you, 
your vendor and possible future vendors.

I have a snap in 120VAC "blade" that goes in as part of a footprint.  The
hole it needs is basically rectangular.

How would one create a pad with a rectangular "hole" in the center?

I know the manufacturer will rout it out with a special bit, but what layer
do I spec that on the Gerbers?  Drill drawing?  Drill guide?  Mechanical?
