IME, an ugly task. There are PDF to DXF converters, and DXFs can be tweaked to Gerberhood, but I've never liked the results. I'm sure that those here who actually do layout day-in, day-out have a better handle, but I can at least say that it's been done.
PDF to DXF can be accomplished using (if nothing better) a GNU proggie called GSView. Look for DXF2PDF on google.
