All, So far, I can confirm that it's "an ugly task."
As it turns out, the nice round pads viewed in Acrobat are octagonal when converted to DXF. Also, rectangular pads aren't filled and I can't seem to make CAMtastic fill them all. I used two PDF > DXF convertors: GSView and Lead Tools Eprint.
It's tantalizingly close, but not close enough. It looks like I could waste a good deal of time continuing on this path.

IME, an ugly task. There are PDF to DXF converters, and DXFs can be tweaked to Gerberhood, but I've never liked the results. I'm sure that those here who actually do layout day-in, day-out have a better handle, but I can at least say that it's been done. PDF to DXF can be accomplished using (if nothing better) a GNU proggie called GSView. Look for DXF2PDF on google.
