Think human trafficking.
Wrestling with this concept is the specialty of the excellent Laura Agustin. Very short version: the word is used only with sex workers who migrate. Other kinds of workers are migrants (if they're working class), or almost no word at all (if they're bond traders).
Her blog: <http://www.nodo50.org/Laura_Agustin/>. Book: <http://www.amazon.com/exec/obidos/ASIN/1842778609/leftbusinessobseA>. My interview with her: <http://www.leftbusinessobserver.com/Radio.html#071115>.
Also I must confess to a personal bias: I really do look down upon sex workers - strippers and escorts and I sure as hell wouldn't want anyone close to me to be in that industry.
Some people feel that way about economists!
That's a real shame because it is an interesting and important subject. Perhaps Michael will relent if we promise to behave ourselves?
I'm the soul of politeness now, unlike my former self when this last came up.
