Given you seem to have covered the basics I don't see how you can be
expected to fix it without Hotmail telling you by what criteria they
are throwing your mail away.

They are also known to throw mail away for antispam reasons and
refusing to explain why.  So you may never get to the bottom of it.

All the time.  Fixed by covering the basics, asking Hotmail what is
wrong, redirecting mail for Hotmail out through another of my
servers (in a different datacentre), and finally telling people not
to use Hotmail.
