Hi Emil, Do you by chance know what version of Mambo you are running. The feature you are looking for is not a template feature but rather a component add-on. I have not worked much with Mambo but I know the stock Poll component that comes with Joomla 1.0.12 can do all these things. FYI, there is no Joomla 2.0 yet, 1.5 is still in beta.
If it was Joomla, I'd set the module position to access by "registered" users only. The Poll component uses cookies or logs the user to prevent ballet stuffing. There is an add-on called PollXT that gives you even more features, but there site has been done for a while now. This might be a good selling point to have them upgrade to the latest Mambo or cross over to Joomla. Short of that Here a work around to do what you want. It's little work but you should be able to follow along. Install and configure phpSurveyor http://www.phpsurveyor.org/ index.php. You can edit a template to create an unbranded white box version. Then use the Mambo menu admin to create a wrapper link to the survey for registered users only. You can even use phpSurveyor to limit who has taken the survey. If Mambo does not support a wrapper menu link then create a content item for registered users only that has an iframe pointing to the survey.
