<HTML>I have a notebook like that too. I've got tabs separating sections for Labwork, MRIs/CT's, Surgery & Procedures, Hospital, Doctor Visits, Miscellaneous, and Important Info (Printouts of particular articles or doctor info).
I asked for copies of all my records from the 15 days I was in the hospital before and after my surgery (cost money), I always ask to at least receive a copy of MRI reports even if I can't get the actual films and I make sure to get copies of all lab work done. Getting copies of past medical records is always a hassle and usually costs a fee, but if you ask your doctor for copies of, say, your bloodwork right when you get it done (or go in for the results), they'll usually just run you off a copy right there in the office. Make sure you let your doctor know that you want copies of labs when they're ordering it because sometimes the doctor has to actually let the lab know ahead of time that it's ok to give you the information. Seeing your lab levels and other test results in print can give you a huge advantage- sometimes you'll actually catch something your doctor missed and you will know exactly what to ask your doctors about.
I also have graphs, created on my computer, of all of my GH, IFG-1 and Prolactin levels, letting me (and my doctors) see at a glance how they've fluctuated since diagnosis and what dates they correspond to. It's not necessary but I find it very interesting to see when my levels were up or down and how they impact each other.
Having this notebook makes me wonder sometimes if I'm crazy or paranoid but it's a huge help when going to a new doctor and a great reference for myself when I read something new about this illness and I can go back to verify information in my own history.
