I have had three surgeries--all transphenoidal.  What kind of surgery is done on you may depend  more on what the surgeon is comfortable and or skilled at doing rather than what is objectively best for the patient.  I had my first surgery by a man listed in the "Top Doctors" book.  His speciality was brain surgery but NOT pituitary surgery.  After the first surgery, he also wanted to do a crainial one for the second one.  Crianal  is much more drastic than transphenoidal.  I left his office and found another surgeon for the second surgery.  For the third surgery, I found one the best there is anywhere for pituitary transphenoidal--Dr. Jho, in Pittsburgh, PA..   Doctors rarely say they can't do a surgery because of a lack of their skills.  They tell you they can't do it because there is something wrong with YOU.    That is my experience.  For what it is worth.  If you have a health plan that is not geographically confining, I say shop around.  Try the PNA site--Pituitary Network of America.   Blessings to you, greg
