11384 cascading failure hides exception during image creation
11692 image upgrade could be more resilient
11776 refresh should be more resilient against unexpected failure

I've tested this (along with various attempts at mutilation) on my own
build 122 system, and it seems to make the image upgrade process a lot
more robust.
Feedback appreciated on the approach (as well as reviews).
