The following webrev contains fixes for the following issue:
9218 pkg.client.api should check for cross-alias/prefix duplicates

This set of changes corrects the API's behaviour in checking for
duplicate publishers by ensuring both alias and prefix are unique across
all publishers both when adding and updating publishers.
Repository origins are intentionally not used as a basis for duplicate
detection.
