This set of changes corrects the API's behaviour in checking for
duplicate publishers by ensuring both alias and prefix are unique across
all publishers both when adding and updating publishers.
Repository origins are intentionally not used as a basis for duplicate
detection.
Should we do something if this condition is already extant - i.e. when
we read the cfg_cache file?
