Here is the cut&paste from the config file:

bochs-opt boot: a
bochs-opt diskc: file=../../483M.NT40, cyl=1024, heads=15, spt=63
bochs-opt cdromd: dev=/dev/cdrom, status=inserted
bochs-opt floppya: 1_44=../../floppya, status=inserted

I created the disk image with dd if=/dev/zero.
