This makes me nervous; in order to create an annotation
you have to modify the metarecord.  This seems as dangerous
as any on-PDA editing.  Updating the original would erase 
all commentaries, even if most records hadn't changed.  It 
also makes it difficult to separate annotations from the 
original document.  One of the proposed uses was multiple 
commentaries on the same original document -- but those 
commentaries would all have to be merged at once, instead 
of just beaming (or synching) an extra commentary.  

If I recall correctly, there are proposals (and maybe
partial patches) for both external links and exact 
anchors.  In the end, I think it might be simpler to
define another document type that (new enough) viewers
will know to look for.  These will contain exact anchors
(with or without a record-level explicit marker about 
which document they refer to) and the annotation itself.

I do like the idea of defining an annotation type record,
so that there is room to say how it should modify the
original -- embed, replace, highlight, popup, etc.
