Run the maildirmake in /var/qmail/bin, and either set the qmail-start's
default delivery mode to Maildir, or make sure each user has a .qmail file
containing "./Maildir/"
You can do this in /etc/skel (or whatever your system uses) and all
subsequently created users will have a Maildir and a .qmail in their
directory.

But note that qmail's POP3 only works with maildir.
