Rich:
PDF Arranger does a great job of extracting pages, which can be printed
to a new pdf file.
I just need to readjust the margins in the new document.
I will take a look at Master PDF Editor.

Two thoughts:
1. To extract a specific page, or range of pages use pdftk.
2. To edit a PDF file use Master PDF Editor. The free version does 99% of
what I need to do; the paid version is much more capable.
