I still can't reproduce the noise on pause.  

To try and narrow down the cause I've built a 'squeezelite-win
1.9.1-1139 that uses the DLLs from the 1.8.4-737 release'
(http://ralph-irving.users.sourceforge.net/files/squeezelite-1.9.1-1139-dll.zip).

The ffmpeg DLLs are not included as they crash squeezelite when an alac
or wma file is played.  That's partially why I moved away from DLLS for
the decoders.
This should not matter as you only mention using flac and mp3.

If you experience the high pitch noise on pause with this build then
it's something that's changed in squeezelite.

If you do NOT then it's something that's changed in portaudio.
