According to the log it looks like Custom Browse thinks that Multi
Library is installed but the errors shows indicates that the tables for
Multi Library hasn't been created in the database.
Do you have Multi Library installed or have you previously had Multi
Library installed ?
Have you tried to restart SlimServer to see if it solves the problem ?

So yours is clearly behaving differently for some reason.
Is the following options shown when you create the menu ?
- Library
- Limit to active library
- Only include when these libraries are active (empty = all)
- Exclude when these libraries are active
If you don't get it to work some other way, you could try to:
1. Install Multi Library and restart SlimServer
2. Unintall Multi Library and restart SlimServer
3. Try to create the menu again
This sequence should create the multilibrary_libraries table, although
this shouldn't be needed and it works in my setup without it.
