There was a problemmatic plugin that overrode the add button; can't remember 
what it was now, but I do remember submitting a patch into SbS code to display 
warnings in the log whenever a plugin tried to override another plugins 
keybinding, and another informational message to display each time a keybinding 
is set by a plugin.

Change the logging level for player.ir to Information, and you'll probably be 
able to work out what's happening.

The log is continually updated on disk.  Perhaps you looked at an old file, not 
the one the new server software is writing to?
