Is this your latest attempt to run the software? The log entries are dated Nov 12th - 8 days ago. It does look like you have some issues with your installation, which will be worth fixing before looking into PlaylistMan. It looks like the database couldn't be accessed, so I'm amazed it works at all. It could be that you have some anti-virus/firewall software that is blocking access to .myi files, which the SbS database tries to write to. You need to exclude this file type for scanning from those apps.
