That is because the r= was made compatible with the other matchers, so 
you can say
=someexe,r=regex,*othermatcher*

The other matchers depend on the case lowering.

To handle your situation, I uploaded a new 4.4.15 complete beta with 
new exe 4.4.15a which allows a single matcher
re=regex
(note re, not r) and which is case sensitive

(You can put the ~ or <min> etc in front if needed)
