I'm inclined to think Dennis is on the right track, concerning SCSI termination.

The FWB Web site indicates that CD-ROM Toolkit v. 3.0.2 and 4.0.1b do support Yamaha CRW4416S, though with this footnote (which I don't understand): "23. If you have a pre-3.0.2 version, you might try using the driver from Mac OS 7.6 (Apple CD-ROM 5.3.1), which supports many drives, just as a test.
