Additional dumb question (#2):
What "Printed By" field? I don't get this and I suspect it has a lot
to do with the printer driver and OS in question. Under OS X and PM
5.0.1, my HP 990cse does not allow for such a header. Nor can I seem to
adjust headers or footers or have a banner page.
Anyway, I hope you solve your problem.
