I saw this interesting discussion on Apple's Carbon list: <http://lists.apple.com/archives/carbon-dev/2008/Aug/msg00148.html> from an Apple engineer: "I have occasionally seen failure-to-hide when the application took an unusually long time to launch -- it bounced in the Dock, and stopped because the Dock (or some other part of the system) gave up on expecting the app to finish launching. When it happened, quitting and relaunching the app always fixed it for me. If you're seeing the same thing I am, I doubt there's anything your app can do about it (unless the reason for very-long launch time is under the app's control)."
