Subject: Powernet: Are Anticipated Dose Rate Alarms Documented in CAP?

 

Robinson would like to determine if other sites document "anticipated
dose rate alarms" which occur after they have been discussed and
documented during a pre-job brief, as "Adverse Conditions" using their
site's Corrective Action Program (CAP).  At Robinson our procedure
requires us to perform a documented "Direct Reading Dosimeter"
investigation for all dose and dose rate alarms, and to also document
all dose and all unanticipated dose rate alarms in CAP.  

 

Does your site document anticipated dose rate alarms in CAP? No.  We use
the electronic shift log to document anticipated alarms.  All
anticipated must be pre-briefed and documented or they are considered
unanticipated alarms.  Drop down provides standard language for the
briefing and for the actual alarm if they occur.

Does your site document unanticipated dose rate alarms in CAP? Yes

Does your site use another tracking system (besides CAP) for tracking
all dose and dose rate alarms? CAP for unanticipated ED dose rate alarms
and all dose alarms.  Shift log for anticipated dose rate alarms
