Robinson would like to determine if other sites document "anticipated dose rate alarms" which occur after they have been discussed and documented during a pre-job brief, as "Adverse Conditions" using their site's Corrective Action Program (CAP). At Robinson our procedure requires us to perform a documented "Direct Reading Dosimeter" investigation for all dose and dose rate alarms, and to also document all dose and all unanticipated dose rate alarms in CAP. Does your site document anticipated dose rate alarms in CAP? No, they are logged in RP logs. Does your site document unanticipated dose rate alarms in CAP? Yes, Does your site use another tracking system (besides CAP) for tracking all dose and dose rate alarms? Yes, Proceduralized investigation form is completed for all alarms.
