Folks,

With regard to the multi-unit/multi-source dose assessment, I would like
to perform a brief survey to see what software solution people intend on
using to establish the capability. This could be useful for future
benchmarking/alignment opportunities, bulk purchasing opportunities,
etc. I will put together the summary of the responses by 5/31, and send
out the results for all to benefit. I appreciate you taking the time to
complete this survey and any additional comments are welcome.

------------------------------------------------------------------------
------------------------------------------------------------------
Licensee Name:

What is your Current Dose Assessment Software Solution? MIDAS
Does the software have Multi-Unit Capability? Yes
Does the software have Multi-Source Capability? Yes, but we have not
implemented this version yet.

If your software does not have Full Capability Dose Assessment have you
identified an alternative software solution?

------------------------------------------------------------------------
-------------------------------------------------------------------
Example filled out survey:

Licensee Name: Southern California Edison (2 units)

What is your Current Dose Assessment Software Solution? Raddose V
Does the software have Multi-Unit Capability? Not full capability. Dose
assessment can be performed currently on each unit then and manually sum
the output
Does the software have Multi-Source Capability: Yes, up to 3 sources per
unit

If you software does not have Full Capability Dose Assessment, have you
identified an alternate software solution? TBD. Potentially upgrade
Raddose V or new software package
