Chris, I use it on a Microtek 4000t film scanner and an Epson 3200 flatbed.
All I can say is that it works well for me. My 4000t was pretty good without
calibration but the Epsons I've used it on exhibit a very strong magenta
cast pre calibration so benefited enormously from the calibration. The slide
for the 4000t came with it and is a kodak stock but I don't know who
manufactured it. The 5X4 slide for the Epson is a Silverfast manufactured
item. Give me a ring if you need any more information.
