I cannot comment on the 2200 specifically, but it would appear from the 1290 driver that it cannot print centrally. If you imagine A4 landscape you have approx 3mm border at the top and approx 14 mm at the bottom to allow the printer to feed the paper. Its idea of printing centrally is to print in the middle of what is left!
With the OSX printer driver it gives an option of formatting for any printer, if you choose this and make custom borders of 14mm top and bottom, I would have thought most printers would understand this instruction. The 1290 does anyway.
Are you sure you are not inadvertently choosing Format for 2200 after specifying your custom paper size? ie can you choose your new custom paper size from the drop down paper size menu????
Stick to format for any printer in page set-up and choose your new paper size with custom borders.
Unless someone knows the 2200 has a specific bug which does not allow format for any printer?
