Welcome to the ProDIG List. Before I try to answer your question, I have changed your subject line to make it easier for following any thread that develops, and in particular for the searching of the archives.
I think that what may be happening is either variations caused by differing apertures or in the case of a person, movement. Using such a technique in a locked down situation, such as a still life, or by creating three differing exposures via Curves/Layers or Camera Raw is entirely feasible, and I have used this technique myself. Using a compression format such as JPEG could easily fail due to the way in which the compression takes place.
I hope that might give you some clues as to where in your case a failure is occurring.
