As this depends heavily on the type of server and software you are running, it's no surprise that you've have few responses. Imagine if you asked, "how do I set the self-timer on my camera" but didn't bother to mention the make and model of your camera?

Are you using a commercial webhost, or is this a DIY situation?

Are you running Apache on a Unix/Linux server? If so there are many free or low cost options available. Look into htaccess/htpassword or the use of a control panel software like Cpanel.

Are you running Microsoft IIS on an NT server? If so, then the setup is completely different. Look into Plesk for one option at a control panel solution.

Are your using Webstar or other on a Mac server?

Ask an incomplete question, get an incomplete answer.  ;-)
