My current thinking is that what is really needed is some kind of
interface between the vehicle computer and the operator.  

Initially, rather than a breathalyzer, there could be some kind of
cognitive/ reaction time test that the driver would need to pass before
the vehicle would start.  This would measure ones ability to operate the
vehicle safely without focusing on any particular kind of impairment.  It
would keep persons out of the driver's seat whose competence to drive is
limited by a prescription drug, a highly emotional state, being overly
tired, or any kind of mental impairment.  It would eliminate the need for
family member to decide when it is time to take away a parent's car keys
because of age related decline.

The ultimate approach would be to have an interface to brain activity
that would allow the driver to move only at speeds consistent with
reaction times, response to environment etc.  A vehicle camera could show
the appearance of the road ahead and hopefully future technology will be
able to determine if the drivers brain is seeing the same picture. A
distracted driver whose eyes are on a cell phone rather than the road etc
would find himself automatically slowing down dramatically.
