I don't add an index on Deleted(), even though I always Set Deleted On. I tried it once, about 50 years ago when this topic first came up <g>, and did not measure any significant speed increase, certainly not enough to justify the added work and complication.
Note that the objective is presumably to have your SQL run quickly. If that happens, who cares what the Rushmore score is?
