BibIndex: author index include/exclude first names
* New config variable CFG_BIBINDEX_AUTHOR_WORD_INDEX_EXCLUDE_FIRST_NAMES
is introduced to govern whether to include or exclude first names when
creating author word index. The default is to include first names, as
it used to be traditionally.
