Not sure that "formal" instructions are really necessary. My own informal isntructions might go something like this:
1. Bend leads just past neckdown to 90 degrees. 2. apply heatsink compound to screw tab (underside of TO-220) 3. Insert component. 4. Solder component. 5. Attach using screw/nut Just make sure that your footprint has an appropriate distance between pads and heatsink screw hole. I see your "footprint my (?might?) vary".
