ISSUE-115 (xhr-referer): XHR does not specify what URL to use for Referer [XHR]

HTML5 specifies this in the Resource Fetch algorithm, but XHR does not use that. This should be specified, and in particular if the XHR is being made with a unique identifier origin it should not send a referer at all.
