XHR does not specify what URL to use for Referer. HTML5 specifies this
in the Resource Fetch algorithm, but XHR does not use that. This should
be specified, and in particular if the XHR is being made with a unique
identifier origin it should not send a referer at all.
XHR now uses HTML5 fetch. This removes some text I had to duplicate from
HTML5 as a nice side effect.
