could you do a lookup with qmail-ldaplookup (probably with -d255)? I think
you missed one of the obvious stock qmail-ldap attributes but it is to
early in the (monday) morining -- my brain is still bootstrapping.

The maildir is needed for qmail-secretary (for moderation and user
verification), if you don't use qmail-secretary /var/tmp should possibly
work. qmail-local does some homedir verification (e.g. the conf-umask or
how ever it is called again comes to mind).
