FWIW I have qmail-smtpd log the following info:
pid, envelope addresses, TCPREMOTEHOST, TCPREMOTEIP and bind-prefix.
bind-prefix is simply a token that is passed via the tcpserver invocation so
that I can associated the log entry with an interface binding.
This is logged at:
o any SMTP failure code - temporary or permanent
o every rcpt to:
o a non-zero exit if it occurs
