If qmail-smtpd rejects a message for any reason, be it local resource
problems, policy (badmailfrom), or just some sort of problem on the remote
end, I'd like to see a log message saying that and giving the reason.
Similarly, I'd like to see a log message of every invalid command sent by
a remote server, particularly including EXPN. I'd also like to see a log
of VRFY requests.

Printing these messages to stdout/stderr would be fine. I'm already
running both tcpserver and qmail-smtpd under cyclog.

One additional thing that would be rather nice to have would be
information in my SMTP log that connects a connection with an e-mail
message, whether that be envelope information or a qmail queue number or
what have you. The purpose would be a point of information to connect an
SMTP session with qmail's normal mail logs.
