Jeremy, I had a problem in the past where the From header was incorrect and the domainkey verification dropped the Email (also without a log message).
Assuming that you have implemented domainkey, I would disable verification and see if that allows you to receive the Email. In the /etc/tcprules.d/tcp.smtp files delete the DKVERIFY variable then qmailctl stop, cdb and then start.
